18 years of age or older with Innovations Waiver Services or State Funded Services

Primary diagnosis of Intellectual or Developmental disability

No violent behaviors including (but not limited to) documented history of yelling, screaming, hitting, kicking, biting, scratching, pinching, or destruction of property. 

Resident of North Carolina

Ability to pass a criminal background check.

Potential member must not require skilled nursing care or care that must be signed off by a nurse. 

Ability to complete and submit all required documentation.

  • Birth Certificate​

  • Guardianship or Power of Attorney (if applicable)

  • Social Security Card

  • Current Medicaid Card

  • Psychological Evaluation

  • Current Medication List from Physician

  • Diagnosis from Physician

  • IEP (if applicable)

  • Vaya's Provider Plan (if applicable)
